Weekly Fatality/Catastrophe Report

This table contains the weekly summaries of fatalities and catastrophes resulting in the hospitalization of three or more workers. Employers must report these incidents to OSHA within eight hours. The summaries below include only preliminary information, as reported to OSHA Area Offices or to States which operate OSHA-approved State Plans. The fatalities listed here include only those that initially appear to be work-related, but exclude fatalities that do not appear to be work-related, such as an apparent heart attack of a sedentary worker. OSHA investigates all work-related fatalities and catastrophes. After OSHA's investigation is complete, these reports will be updated with inspection results and citation information.

12/19/2010	United Crane and Rigging, Ft. Meade, MD 20755	Worker was removing packing material from a large battery supply unit using an all-terrain fork truck and the unit fell on the worker.
12/24/2010	Lincoln Manufacturing Inc., Magnolia, TX 77354	Worker was operating a pipe threading machine when he stepped into a scrap metal dumpster and hit his left upper leg against the edge of the dumpster.
1/5/2011	City of Rainier, Rainier, OR 97048	Police Chief shot and killed while attempting to apprehend someone breaking into a car.
1/7/2011	Universal Laundry and Linen, Las Vegas, NV 89101	Worker was discovered lying on the ground and unresponsive. Cause of death was determined to be cardiac arrest, but the worker exhibited signs of electrical burns.
