Weekly Fatality/Catastrophe Report

This table contains the weekly summaries of fatalities and catastrophes resulting in the hospitalization of three or more workers. Employers must report these incidents to OSHA within eight hours. The summaries below include only preliminary information, as reported to OSHA Area Offices or to States which operate OSHA-approved State Plans. The fatalities listed here include only those that initially appear to be work-related, but exclude fatalities that do not appear to be work-related, such as an apparent heart attack of a sedentary worker. OSHA investigates all work-related fatalities and catastrophes. After OSHA's investigation is complete, these reports will be updated with inspection results and citation information.

12/15/2010	Bellevue Community Schools, Bellevue, IA 52031	Worker slipped on ice and hit head on a building.
1/6/2011	Newell Recycling of Atlanta Inc., East Point, GA 30364	Worker was operating a front-end loader removing scrap metal when a spark caused an ignition of gasoline. The fire engulfed and burned the loader with the worker trapped inside.
1/9/2011	Howard Industries Inc., Ellisville, MS 39437	Worker was electrocuted while performing tests on voltage up to 14,440 volts at a transformer testing station.
1/10/2011	J & J Ag Products Inc., Clewiston, FL 33440	Worker was electrocuted while spraying a broccoli field when he raised the tractor's spray boom and hit an overhead power line.

1/19/2011	Key Energy Services, Boyd, TX 76023	Worker was checking drilling collars to determine which ones were defective and was crushed by two drilling collars that had rolled off the stack.
1/19/2011	Rice Logging Inc., Dallas, OR 97338	Worker drove a log loader off a bridge and drowned in the creek/river below.
1/21/2011	Baxter Healthcare Corporation, Los Angeles, CA 90039	One worker was killed and two injured when they were overcome by fumes; two were cleaning a tank, the third attempted rescue.
1/24/2011	Tortilleria Chinantla, Brooklyn, NY 11211	Worker was caught in the auger of a dough mixing machine.
