Weekly Fatality/Catastrophe Report

This table contains the weekly summaries of fatalities and catastrophes resulting in the hospitalization of three or more workers. Employers must report these incidents to OSHA within eight hours. The summaries below include only preliminary information, as reported to OSHA Area Offices or to States which operate OSHA-approved State Plans. The fatalities listed here include only those that initially appear to be work-related, but exclude fatalities that do not appear to be work-related, such as an apparent heart attack of a sedentary worker. OSHA investigates all work-related fatalities and catastrophes. After OSHA's investigation is complete, these reports will be updated with inspection results and citation information.

1/17/2011	Burkshire Construction Company, Highland Heights, OH 44122	Worker was operating an articulating boom lift and was re-positioning the aerial lift when his head got caught between the safety railing and a concrete beam.
1/17/2011	Precision Concrete Steel & Manufacturing LLC., Claxton, GA 30417	Worker was working on a flat metal roof and fell through a flush-mounted translucent skylight panel to the lower concrete surface.
1/20/2011	Acme Markets, Newark, DE 19702	Worker was operating a forklift to load truck trailers at a loading dock and was found slumped over the side of the forklift.
1/20/2011	Flintstone Paver Installations LLC., Buford, GA 30518	Worker was using a 5-gallon tote of gasoline to prepare to burn wooden pallets and was smoking. Part of the smoking apparatus sparked onto the gasoline igniting an instantaneous flash fire.
