Weekly Fatality/Catastrophe Report

This table contains the weekly summaries of fatalities and catastrophes resulting in the hospitalization of three or more workers. Employers must report these incidents to OSHA within eight hours. The summaries below include only preliminary information, as reported to OSHA Area Offices or to States which operate OSHA-approved State Plans. The fatalities listed here include only those that initially appear to be work-related, but excludes fatalities that do not appear to be work-related, such as an apparent heart attack of a sedentary worker. OSHA investigates all work-related fatalities and catastrophes. After OSHA's investigation is complete, these reports will be updated with inspection results and citation information.

11/18/2009	Welty Homes, Inc., Bethesda, MD 20817	Worker either fell from ladder or roof.
12/5/2009	SUNY Binghamton University, Binghamton, NY 13902	A professor was assaulted while providing mentoring services in an assigned office space. Event occurred during normal office hours. Multiple stab injuries to abdomen and torso.
1/7/2010	NYCHA Edenwald Houses, Bronx, NY 10466	Worker of New York City Housing Authority was found dead at his work station in the boiler room. (Inspection planned)
1/18/2010	Cinderella Properties, Auburn, WA 98002	Skull fracture.
1/19/2010	Oak Express, Syracuse, NY 13224	Worker was unloading a trailer, and a box fell on him.
1/20/2010	NYS Police Troop C, Morris, NY	Trooper collided with a westbound tractor trailer.
1/26/2010	NYC Department of Sanitation - Queens West 1, Astoria, NY 11106	Worker was killed after being pinned between a tractor trailer and his garbage truck.
1/29/2010	Capital Trading LLC, West Jordan, UT 84088	Worker was crushed between a delivery truck and the loading dock resulting in serious injuries and his death.

1/29/2010	Kitchell Contractors, Inc, Phoenix, AZ 85106	Worker was run over by a forklift on a construction site and fatally injured.
1/29/2010	Parkdale Mills, Belmont, NC 28502	Worker was in the process of unloading a truck when the forklift he was operating overturned.
1/30/2010	Octopus Car Wash, Albuquerque, NM 87113	A car being driven off of the exit-end of the car wash conveyor to the drying area outside of the building accelerated suddenly, striking the worker standing near the auto door to the outside.
1/30/2010	Sturdivant Woodyards, Inc., Sandersville, MS 39477	Worker was found at the log loading site beneath the front right tire of a skidder.
2/2/2010	Manheim's San Francisco Bay Auto Auction, Hayward, CA 94544	Worker was found lying dead on roadway adjacent to work. Apparent homicide.

2/2/2010	Stone Crafter, Wintor Park, FL 32792	Worker was assisting in the delivery of granite slabs using a forklift. The forklift hit two slabs that fell, striking the worker on the head.
2/3/2010	Apac Southeast, Inc., Winter Haven, FL 33884	Worker was in the process of putting equipment on a machine and was found on the ground by other workers; apparent heart attack.
2/3/2010	P.A.M. Transport, Inc., North Jackson, OH 44451	Worker was found unresponsive after pulling a tractor trailer into the shop building.
2/3/2010	Palatka Welding Shop Inc., Palm Coast, FL 32164	Worker was installing metal roofing and fell 35 feet, hitting the concrete floor.
2/4/2010	Long Foundation Drilling Co., Cullman, AL 35057	Worker was lowered into a 30- to 35-foot deep shaft to perform hand cleaning and to drill a sample test of a rock. He was being lifted out of the shaft by the use of a pneumatic winch system. The winch system collapsed into the shaft, striking the worker.

2/5/2010	Breeze National Inc., New York, NY 10027	Worker fell from scaffold into a window opening and 20 feet down an elevator shaft.
2/6/2010	Green Circle Bio Energy, Inc., Cottondale, FL 32431	Worker received severe steam burns to his lungs during the clinker material clearing process.
2/13/2010	Dynamic Wireless, Haskell, AR 72012	Worker was disassembling a communication tower and fell 98 feet to the ground.
