Weekly Fatality/Catastrophe Report

This table contains the weekly summaries of fatalities and catastrophes resulting in the hospitalization of three or more workers. Employers must report these incidents to OSHA within eight hours. The summaries below include only preliminary information, as reported to OSHA Area Offices or to States which operate OSHA-approved State Plans. The fatalities listed here include only those that initially appear to be work-related, but exclude fatalities that do not appear to be work-related, such as an apparent heart attack of a sedentary worker. OSHA investigates all work-related fatalities and catastrophes. After OSHA's investigation is complete, these reports will be updated with inspection results and citation information.

3/9/2011	Estis Well Service, LLC, Plaquemine, LA 70764	Working was positioning a pipe into a hole on a derrick when the pipe shifted, striking the worker, and pinning him between the rig and mud tank.
3/10/2011	General Supply & Services, Inc., Pompano Beach, FL 33064	Worker was unloading a truck in the parking lot and was run over by a delivery truck.
3/10/2011	Georgia Power Company, Claxton, GA 30458	Worker was climbing down a power line transmission structure when he fell to the ground.
3/10/2011	Ultra Building Services, The Woodlands, TX 77381	Worker operating an electric floor buffer in the building was found unconscious and non-responsive by building security.
3/11/2011	Westfalia Technologies, Inc., Tampa, FL 33612	Worker was replacing chains on a storage retrieval machine when a vertical lift station came crashing down, striking the worker.
