Weekly Fatality/Catastrophe Report

This table contains the weekly summaries of fatalities and catastrophes resulting in the hospitalization of three or more workers. Employers must report these incidents to OSHA within eight hours. The summaries below include only preliminary information, as reported to OSHA Area Offices or to States which operate OSHA-approved State Plans. The fatalities listed here include only those that initially appear to be work-related, but exclude fatalities that do not appear to be work-related, such as an apparent heart attack of a sedentary worker. OSHA investigates all work-related fatalities and catastrophes. After OSHA's investigation is complete, these reports will be updated with inspection results and citation information.

3/4/2010	University of Wyoming, Laramie, WY 82071	A material handler was walking in a parking lot when he slipped on some ice causing him to fall, striking his head on parking lot surface.
3/5/2010	Big Boyz Truck, Motley, MN 56466	Worker was working beneath a trailer when the jack stand broke.
3/6/2010	Valor Technologies, Inc., Mt. Prospect, IL 60058	Worker was disposing waste parquet flooring and tile from a gondola plastic cart into a dumpster. Worker collapsed into the dumpster; apparent heart attack.
3/10/2010	Dave Shaw Concrete & Block Inc., Hermosa Beach, CA 90254	Worker was standing on platform/plywood on forklift and fell into a 15-foot excavation which collapsed around him, burying him.
3/10/2010	Etchegaray Farms, McFarland, CA 93250	Worker was struck by falling windmill blades when he went to start equipment.
3/11/2010	AEE Tecnica de Rio Piedras, Santurce, PR 00908	Worker was hit in the forehead by the hook of a crane that was being used to install a utility pole. He was knocked unconscious and transported to the hospital where he later died.
3/12/2010	GATX Corporation, Camp Minden, LA 71058	Worker had climbed down a ladder into a railcar, which had been identified as a confined space, wearing an acid gas respirator with an organic vapor cartridge. As he began to exit the railcar, climbing up the ladder, he collapsed and fell back into the railcar. It is believed there was a nitrogen atmosphere in the railcar.
