This part of the weekly FAT/CAT Report summarizes preliminary information provided to OSHA from employers, which are required to notify OSHA of fatalities or catastrophes (FAT/CAT) in accordance with OSHA standard 29 CFR 1904.39. After incidents are investigated by OSHA, information from these initial employer notifications is updated if necessary. All incidents are assigned an event number (OSHA-36 Form) by OSHA's local Area Office. Subsequent inspections by OSHA are assigned an inspection number (OSHA-170 Form).
