Weekly Fatality/Catastrophe Report

This table contains the weekly summaries of fatalities and catastrophes resulting in the hospitalization of three or more workers. Employers must report these incidents to OSHA within eight hours. The summaries below include only preliminary information, as reported to OSHA Area Offices or to States which operate OSHA-approved State Plans. The fatalities listed here include only those that initially appear to be work-related, but exclude fatalities that do not appear to be work-related, such as an apparent heart attack of a sedentary worker. OSHA investigates all work-related fatalities and catastrophes. After OSHA's investigation is complete, these reports will be updated with inspection results and citation information.

10/11/2010	Rose Hill Conference Center Nashville, NC 27856	Worker was operating a small tractor that overturned and pinned him underneath.
10/13/2010	BAE Systems, York, PA 17408	Worker was operating an aerial lift and got caught between the lift and an overhead door's overhang.
10/15/2010	Willoway Nurseries Inc., Huron, OH 44839	Worker was operating his personal vehicle at a railroad crossing and was struck by a train.
10/16/2010	(Company name not determined) Long Beach, CA 90603	Fisherman was electrocuted on a boat at sea as he was changing a capacitor in a breaker box. (U.S. Coast Guard is investigating.)
10/17/2010	Haberle Steel Inc., Souderton, PA 18964	Worker was crushed by a section of steel plate.
10/18/2010	ArcelorMittal, Vinton, TX 79835	Worker was lighting up a gas furnace. An explosion occurred and the worker was struck on the head by falling debris.
10/19/2010	Bush & Burchett, Inc., Catlettsburg, KY 41129	Worker was measuring a grade for the bridge brackets, outside the life-line system, on the I-64 bridge when he fell approximately 30 feet to the ground below. He was wearing a fall protection harness, but was not tied off.
10/20/2010	Federouch Landscape Supply, Carnegie, PA 15106	Worker without fall protection was replacing sections of a roof. He fell through a 40-inch opening in the roof and down 31 feet to the ground.
10/20/2010	Roy Case Construction, Mountain Pine, AR 71956	Worker was operating crane when crane tipped. Worker fell and was crushed between crane cab and a wall.
10/21/2010	Qualico Steel Company, Inc. , Webb, AL 36376	While changing and wiring two heaters, worker turned on electricity to check wiring. He forgot to turn off the electricity and was electrocuted by 480 volts upon contact.
10/21/2010	Wright Brothers Construction Company,Inc., Charleston, TN 37304	Worker lost control of an articulating dump truck at a landfill. The truck flipped over, pinning the operator under the motor compartment.
10/22/2010	Alliance Electric LLC, Harpers Ferry, WV 25425	Worker was holding end of the hose while pouring concrete from pump truck. Truck's boom contacted overhead line, electrocuting worker.
10/22/2010	Grayco Construction Co. of TN, LLC, Conway, AR 72032	Worker had parked a water tank truck on an incline. While filling the truck's tank, the truck rolled down the incline and over the worker.
10/23/2010	TKH Northeast, Inc., Hornell, NY 14843	Worker was operating an earth roller that tipped and crushed him.
10/23/2010	Universal Plumbing, Augusta, GA 30904	While worker was uncovering a sewer pipe with a shovel in a trench approximately 6 feet deep when the trench collapsed.
