Weekly Fatality/Catastrophe Report

This table contains the weekly summaries of fatalities and catastrophes resulting in the hospitalization of three or more workers. Employers must report these incidents to OSHA within eight hours. The summaries below include only preliminary information, as reported to OSHA Area Offices or to States which operate OSHA-approved State Plans. OSHA investigates all work-related fatalities and catastrophes. OSHA will link the data on this page to the inspection case file, also on the OSHA public website.

8/11/2009	CA, Transportation, Richmond, CA^* 94801	While taking tolls, a person drove up and shot the worker in the face.
8/25/2009	Speed Link, Orinda, CA^* 94563	The worker was found in his vehicle by the Orinda P.D. and was unresponsive.
9/4/2009	Accent Care, Walnut Creek, CA^*94595	Worker was found face down on site and taken to the hospital. Employer was informed that worker died on 9/7/2009.
10/15/2009	SF American Taxicab, Inc., Orinda, CA^*94563	Worker's vehicle was clipped by another vehicle causing the worker's vehicle to hit an on-coming big rig. The impact between the worker's vehicle and the big rig caused the fatality.
11/19/2009	Beyond Green, Inc., Paddock Lake, WI 53168	Worker working in a tree, fell hitting their head.

11/23/2009	Z-Medical Inc., Irvine, CA^* 92606	Worker got into an accident, got out of the car, walked onto the street, and was hit by another car.
11/24/2009	Johnson Controls Inc., Pulaksi, TN^* 38478	Worker was adjusting the temperature leaning through a guardrail. He was struck by a roller cam, and his head was caught between the guardrail and roller cam.
11/24/2009	Roseburg Forest Products Co., Riddle, OR^* 97469	Worker was on a platform by a sander. The platform gave way, and the worker fell to the concrete floor.
11/27/2009	Dix Hills FD Headquarters, Dix Hills, NY^* 11746	Worker suffered a stroke in his home on 11/21/2009. Worker responded to alarm earlier that same day. Worker died at the hospital on 11/27/2009.
11/28/2009	Eastern Illinois University, Charleston, IL^*	While riding home from a football game, the worker was killed in an automobile accident.
11/30/2009	AAA Planta Regional de Caguas, Caguas, PR^* 00725	Worker fell in underground pit tank where there were recirculating pumps.
11/30/2009	King FIsher Marine Service, Inc., Port Lavaca, TX 77979	Worker was painting a boat on the water and was unaccounted for. Worker drowned, and the body was found underwater.
11/30/2009	Naperville School District 203, Naperville, IL^*	Worker gained access to the roof of the construction area. The worker fell from a significant height and died from those injuries. It is unclear whether the worker jumped or fell from the roof.

12/1/2009	Salem Golf Culb, Salem, OH 44467	Worker was cutting down a tree using a chain saw. The tree fell onto another tree, causing a branch to fall on the worker.
12/1/2009	United Industrial Services, Chelmsford, MA 01824	Worker was preparing to remove waste containers from the highway and was struck by a motor vehicle.
12/1/2009	Viking Electric, L.L.C., Phoenix, AZ^* 85034	Worker was changing the ballast on a light fixture and was electrocuted.
12/2/2009	Raymond F. Kravits Center for Performing Arts, West Palm Beach, FL 33401	Worker was installing stage lights at a height of 25 feet from a truss system and fell.
12/3/2009	Kid Jones Oil Co., Tyler, TX 75701	Worker was standing on the medium and was struck by a car.
12/4/2009	Valero Refinery, Texas City, TX 77590	Worker was lighting a pilot on a boiler in a stream unit when the boiler catastrophically failed.
